MSK Community Link Specialist
37 hours per week, salary £19,240 working in Mid Sussex, Horsham and Crawley
Fixed term contract to September 2016
Musculoskeletal (MSK) conditions cover about 200 different conditions affecting the muscles, joints and skeleton. Disability due MSK disorders is increasing due to ageing of the population and accounts for up to 30% of GP visits in England.

They are delighted to be working with other voluntary sector partners in West Sussex to deliver a pilot up to September 2016. They will establish working relationships, systems and practice to support patients and carers in accessing appropriate self-management tools and approaches and supporting them to manage their conditions.
They want to cultivate working relationships that bring the skills and knowledge of organisations and professionals together in partnership to improve the experience and outcomes of patients. This role will be working with an identified cohort of patients, health professionals and the voluntary sector to understand the barriers to self-management and co-design potential solutions.
They are looking for an enthusiastic  individual(s) to join the team with proven communication and people skills and knowledge and experience of delivering a service that requires self-motivation, research and development.

Housing Solutions Coordinator
37 hours per week, salary £19,240 pa based in Littlehampton working in Chichester and Arun
They deliver an information and advice service to people aged over 50, their families and carers on a wide range of issues, operating an AQS quality marked advice line from their headquarters in Littlehampton, face to face advice at outreach locations throughout West Sussex and home visits for less mobile clients
They are extending this very successful service to include providing a move management service for those clients who decide their best option is to move but they do not have the network of support to enable them to realise their preferred housing solution. The support provided will include help with paperwork, accessing professional services and supporting the client to move from one home to another.
They are looking for an enthusiastic individual(s) to join their team with proven communication and people skills and knowledge and experience of delivering advice.
This post needs you to have a full driving licence and use of a car.

Customer Service Adviser, Help at Home
20 hours per week, salary £9,204 pa based in Littlehampton
They are looking to recruit a Customer Service Advisor to support their team delivering the Help at Home Service covering cleaning, shopping and a range of household tasks. The role will involve visiting customers to determine their requirements, assessing their needs, and matching self-employed home helps to deliver the service. They also monitor the service to ensure quality standards are maintained. You need to be an organised person with good communication, observation and administration skills. This post needs you to have full driving licence and use of a car.

Senior Coordinator, Home from Hospital
32 hours per week, salary £15,458 pa based in Littlehampton
The Home from Hospital service provides older and vulnerable people with low level support for a limited period after hospital discharge. The purpose of the service is to ensure people have a successful hospital discharge by accessing help and support and regaining their confidence to ensure they are able to return to independent living.
The Senior Coordinator will work with the manager of the service and be responsible for a team of four coordinators covering West Sussex. Carrying out customer assessments, developing personal support plans, matching customers with volunteers and monitoring progress to ensure a successful outcome.
You need to be an organised person with good administration, observation and communication skills. Their aim is to recruit an enthusiastic individual who enjoys working with people to enable them to adopt an independent lifestyle.
This post needs you to have a full driving licence and use of a car.
